An End-of-life Care Symptom Response Kit has been developed for in-home palliative clients who could require unanticipated symptom management or for those with rapidly escalating symptoms. Pre-ordering of this kit by the physician allows for greater flexibility of symptom management for the visiting community nurse, contracted by the Waterloo Wellington Community Care Access Centre and may avoid visits to the Emergency Department.
